How to Nominate
Eligible entries are for Junior Athlete of the Year, Leadership Awards (3), Corporate Excellence Award and the Spirit of Sport Story of the Year – accomplishments, stories or contributions that took place between January 1, 2011 and November 30, 2011, except for lifetime achievement awards. Any person that meets the current Canadian Sport Awards eligibility criteria may be entered for consideration. Please review and comply with the 39th Canadian Sport Awards guidelines.
Nominations for four of the five Athlete Performance Awards (Male, Female, Team and Partners of the Year) will be populated by the TOP 8 results published by Sport Canada and from other relevant sources. The Selection Advisory Committee will determine, based on those results, the top 3-5 in each category. Those top 3-5 nominations are then sent to the jury for ranking 1-3. Therefore no nominations are required for these award categories (Male, Female, Team and Partners of the Year).

Nominations must be accompanied by all relevant information, a biography and colour photograph of the nominee, and a maximum of three relevant media articles per nomination.

Hard copy versions may be submitted by mail to 350-955 Green Valley Crescent, Ottawa, Ontario, Canada K2C 3V4 - Attention: CSA Nominations.
Deadline
Nominations must be submitted to the True Sport Foundation by 5:00 p.m. EST Monday, December 19, 2011, and late submissions will NOT be accepted. Nominators will be notified by email or phone if their nomination is incomplete. Nominations can be re-submitted until the specified deadline.
